Your new company
This Federal Government Department deliver free and confidential nation-wide counselling and support for service-related mental health conditions. They are now seeking a Complex Case Manager (Clinical) (Psychologist, Social Worker, Registered Nurse, Mental Health Registered Nurse, Occupational Therapist) to join the Well-being and Support Program in a full-time capacity on a 12 month initial contract.
The WASP provides intensive psycho-social case management for veterans with complex health needs who have recently transitioned or have exceptional needs which would benefit from psycho-social case management.
Your new role
In your new role, you will utilise experience gained in case management settings and/or clinical settings, working with clients with mental health conditions, vulnerabilities and/or complex co-morbidities. You will be required to be experienced in considering the social determinants of health and wellbeing, and using the domains of wellbeing identified in the model, apply this knowledge to all aspects of case management practice including case reviews. Other responsibilities will include:
Working collaboratively with key stakeholders to achieve outcomes for clients, building and sustaining relationships with internal, external and inter-departmental stakeholders.
Provide an escalation point for clients at risk of harm to self or others.
Provide specialised case management and/or clinical expertise and advice to support and enhance the work of the Complex Case Managers, and manage a case load.
Undertake a range of client assessments including intake assessments and risk assessments.
Contribute to the preparation of high level client summaries and briefs, including maintaining high quality documentation.
