About 8:45am on December 16, 2020, emergency services were called to Gundaroo Road at Gunning, following reports two Mazda 3 sedans had collided head-on.
It's alleged the driver of a grey Mazda 3 failed to negotiate a right-hand bend and collided head-on with a white Mazda 3 sedan.
The driver, a 33-year-old man, and a male passenger in the grey Mazda and two women in the white Mazda were taken to Canberra Hospital with serious injuries.
Officers from Monaro Police District attended and commenced an investigation into the circumstances surrounding the crash.
Following further inquiries, the 33-year-old man - disqualified from driving for life in NSW and ACT - was extradited from the ACT on January 20, and taken to Queanbeyan Police Station.
The man appeared in Queanbeyan Local Court on January 21.
